This application offers users streamlined access to their health plan and medical records, alongside essential resources tailored for members, patients, and their families.

Key features of the medical record include:

• Reviewing test results, medications, immunization histories, and more.
• Maintaining communication with your physician.
• Managing appointments, including e-Visits for participating providers.
• Uploading health and fitness data, including information from the Apple Health app, while enrolled in self-tracking programs.
• Accessing your family’s health information.

Access to these features requires a Community Medical Centers MyChart account. Users without an account can sign up within the app.

Health plan features offered include:

• Viewing and sharing your health plan card.
• Searching and reviewing your claims.
• Accessing detailed benefits information.

Similar to the medical features, to utilize these capabilities, users will need to log in with their Community Care Health Member Portal account.
